EveryDay Care & Support: Collaborative Services Improving Life for Older Adults.

Since being established, EveryDay Care & Support’s vision has been guided by the values of Age UK North Tyneside. EveryDay was developed to become an extension of the services through the charity with multiple care options, personalised care plans and holistic support offered through a single point of contact.

Strategically Working Together

As soon as someone is introduced to one service, a door is opened to all services from both EveryDay and Age UK North Tyneside. This is thanks to the strategic working of both organisations.

We work collaboratively and cohesively to develop, change, and align our services to benefit older people. Not many other care companies do it like that.

Supporting Older People from Hospital Discharge to Home

One case of our exceptional collaboration is the way we supported Mr F. He was admitted to the hospital following a fall, and then put into Step-Down Care – a place that bridges the transition of hospital discharge to getting back home.

Once back in his own home, Mr F was referred into Age UK North Tyneside’s Care Point service. Here, he was visited by our Promoting Independence Coordinators, where with the customer, they created a 12-week plan for making sure he had everything he needed to be back at home. This looks at not only his physical needs but at reconnecting with the community and putting things in place so he can continue to live independently at home.

Now, Mr F has a personalised care plan with EveryDay Homecare, where community-based care helps with keeping him at home, surrounded by family and now taking part in activities run by Age UK North Tyneside, including Strength & Balance, where he can work on his mobility, strength, and coordination.

Having a whole support package and points of contact if he needs, Mr F can now be confident that he has everything he needs to stay at home with independent living support. A priority for us at EveryDay.
